How to Obtain Copies
OPA is pleased to provide paper or CD copies of medical records and images to authorized individuals or groups with proper signed release and consent. Records Brought to OPA from Outside Sources.

OPA will receive and store the records and images patients bring to us from outside sources for reference purposes.  The law prohibits medical providers from releasing records to third parties that were obtained from other outside offices.

The Medical Record
OPA Medical records consist of the following different items:

• The chart note (a summary of your visit with the doctor and his/her findings).
• Operative reports.
• Lab and pathology reports.
• Imaging studies (x-rays, MRI, CT, Bone Scan, etc.).
• Emergency Room and other hospital reports.
• Medications issued.
• Physical therapy notes.
• Reports from other consulting or referring physicians
