Health Records and Images
 
  Patient personal health information including medical records, X-Ray and other images are the property of OPA and we are required to store and safeguard this information in accordance with the law.  Patients can be assured that their records and images will NOT be released by OPA to any unauthorized individual or group.  Workers Compensation, patient’s insurance carriers, including government carriers, may legally access personal health information without patient consent.

How to Obtain Copies
OPA is pleased to provide paper or CD copies of medical records and images to authorized individuals or groups with proper signed release and consent. Click the links below for detailed information, policies and instructions to obtain copies of records and images for yourself, another medical provider, an attorney or a minor child.

Records Brought to OPA from Outside Sources.

OPA will receive and store the records and images patients bring to us from outside sources for reference purposes.  The law prohibits medical providers from releasing records to third parties that were obtained from other outside offices.

The Medical Record
OPA Medical records consist of the following different items:

  • The chart note (a summary of your visit with the doctor and his/her findings).
  • Operative reports.
  • Lab and pathology reports.
  • Imaging studies (x-rays, MRI, CT, Bone Scan, etc.).
  • Emergency Room and other hospital reports.
  • Medications issued.
  • Physical therapy notes.
  • Reports from other consulting or referring physicians
